Social Work / Case Management / Utilization Review
A Spectrum of Services
Bayshore’s social workers are part of the Case Management Department, which helps patients, families, and doctors obtain prompt access to our spectrum of health and wellness services. The social worker helps individuals and families cope with the impact of illness and hospitalization, plan for post-hospital care, and handle physical/emotional/social problems at home. The social worker offers practical help and advocacy in locating appropriate resources, equipment, referral sources, or aftercare facilities and helps in coordinating hospital and community services on your behalf. The social worker also provides counseling services to help you and your family talk over social and emotional concerns and explore solutions related to your healthcare needs.
The social worker will see you privately to discuss your needs. If appropriate, with your approval, the social worker will speak with your family and your doctor to obtain further information about your needs and concerns and help you handle them. If you need help from a social worker, ask your nurse, or call Social Work at 732-739-5964 or extension 5964 between the hours of 8:30 a.m. and 4:30 p.m.
